For Students that are leaving KIS, this form is for you to have the MDM and Self Service removed from your device. If you have multiple devices, please fill in the form for each device. After you submit the form, your device will be removed from the service from the server. Mr. Schultz will remove this as soon as possible. This means that you do not need to go to IT or Edtech to have this done.
